HouseCalls Case Management

MSQLP’s social worker visits the homes of new clients to assess 8 areas of a client’s life: healthcare, social support, finances, functional abilities, activities of daily living, home environment, community services, and perceived future needs. The client and social worker together develop a treatment plan that identifies client objectives which, when met, will improve the client’s quality of life. Our social worker works with the client to meet each of the objectives. Here’s some data on the growth of the case management service we provide.

Support Groups

MSQLP offers three support groups in our local area. Each group is open to anyone with MS and we encourage family members and caregivers to attend. Each support group is peer-led and there is no fee. Each offers a safe, confidential, and constructive place to talk with others who share the commonality of living with MS.

  • listen to and encourage one another
  • share the challenges and victories of living with MS
  • learn new coping skills
  • share ideas and resources
  • plan and bring into being social activities, outings, and educational events

You are invited to come and make new friends and discover what others are doing to improve the quality of their lives.
